TRANSITIONS Psychiatric Center for Older Adults
As those we love grow older, their lives should be as joyful and rewarding as possible. But for some, growing older means losing the ability to live in the same independent way they once did. It means losing certain physical or mental capacities or watching loved ones pass away. In some cases, the stress of growing older can be so overwhelming that it actually becomes disabling making performing even the simplest day-to-day activities nearly impossible. At this point, professional help is needed to bring that person back into balance - back to healthy living. This is where TRANSITIONS at Summit Hospital can help.
TRANSITIONS offers a fully integrated and professional program that is designed to help both aging individuals and their families. Our program offers people ages 55 and older the emotional, medical and psychiatric support they need in order to regain as much of their personal independence as possible.
The Program
TRANSITIONS provides a caring, supportive environment in which each patient participates in a therapeutic treatment program that is specifically designed to meet their needs. On admission, each patient undergoes a comprehensive medical and psychiatric evaluation. Based on these evaluations a structured plan of individual counseling, group therapy, therapeutic activities, medication management and family support are provided. The goal is to regain the social, physical and emotional skills once enjoyed.
The Treatment Team
Providing this treatment and support is the TRANSITIONS Treatment Team. Comprised of psychiatrists, Master's level social workers, nurses, therapists, and other mental health professionals, they all work together to create an integrated and personalized healthcare program.
Continuing Care
Preparing and planning for discharge begins when a patient first arrives and continues as treatment progresses. In this way, we assure that a well-structured, effective aftercare plan is designed for each patient and the family to provide continued support and assistance as needed.
The Support System
The problems of aging affect not only the patients themselves, but also their families. In fact, one of the most difficult dilemmas for the family is acknowledging that the deteriorating health of an aging loved one requires professional help. Facing that fact can cause guilt and pain that are as real as the illness itself.
An essential part of TRANSITIONS at Summit Hospital is assisting family members with their emotions. Family support groups are offered to encourage family members to talk about their concerns, and to show them they are not alone. In addition, we encourage family members to participate in their loved one's treatment program to enable them to better understand the emotional and physical problems of the patient.
Admissions & Referrals
Referrals to the TRANSITIONS Inpatient Program may be made by family members or healthcare professionals.

Treatment services for patients who qualify for admission to TRANSITIONS are covered by Medicare and most major insurance companies.
